Requesting Proof of Insurance Letters

Snohomish County Departments needing proof of insurance letters for outside agencies should provide the following information to Risk Management: 
  1. Name of agency, mailing address, and the contact person's name and title 
  2. Reason for the request. If it’s for a specific event, please provide:
    1. The date(s) of the event
    2. The location of the event
  3. Instructions on the delivery method (mail, email, pickup)
