MEDICAID ALTERNATIVE CARE (MAC) AND TAILORED SUPPORTS FOR OLDER ADULTS (TSOA) CASE MANAGEMENT 

*NEW* Snohomish County Long Term Care and Aging is soliciting responses from interested and qualified organizations to submit responses for the contract to provided Medicaid Alternative Care (MAC) and Tailored Supports for Older Adults (TSOA) Case Management services. This contract will provide case management services to vulnerable persons, age 55 or over and their unpaid caregiver who receive pre-Medicaid funded Long Term Care Services and Supports in their homes through the MAC or TSOA programs. The period of performance is August 1, 2019, through December 31, 2019. Please refer to the documents below for the qualifications required, timeline for the RFP process, response instructions, and the Basic Terms and Conditions for Snohomish County.

A Letter of Intent to apply for the RFP must be submitted by 4:00 p.m. local time, Friday, June 7, 2019. Organizations not submitting a Letter of Intent by due date and time will forfeit their right to apply for RFP.

BEHAVIOR HEALTH-PREVENTION SERVICES

Snohomish County Human Services Long Term Care and Aging is soliciting proposals from interested and qualified organizations to allocate one-tenth of one percent sales and use tax funds for a contract(s) for Prevention Services for Behavioral Health for older adults, 55 years of age and older, living in Snohomish County. Contract amounts entered into as a result of this RFP will be negotiated with the successful RFP candidate(s). The anticipated contract(s) will be awarded July 1, 2019. The County reserves the right to award additional contracts without resorting to the competitive RFP process for contractor(s) having successful annual contract monitoring reports. Additional contract(s) funding will not exceed a total of $100,000 on an annual basis. There is no guarantee of ongoing funding.
