Request for Proposals (RFP)

Medicaid Alternative Care (MAC) and Tailored Supports for Older Adults (TSOA) Case Management


Snohomish County Long Term Care and Aging is soliciting responses from interested and qualified organizations to submit responses for the contract of Medicaid Alternative Care (MAC) and Tailored Supports for Older Adults Case Management. This contract will provide case management services to vulnerable persons, age 55 or over and their unpaid caregiver who receive pre-Medicaid funded Long Term Care Services and Supports in their homes through the MAC or TSOA programs. The contract amount is fixed at $100 per client per month. The amount is subject to appropriation. 


The period of performance is July 1, 2020, through December 31, 2020. Please refer to the Request for Proposal Information below for the qualifications required, timeline for the RFP process, response instructions, Basic Terms and Conditions, Business Associate Agreement and the proposer’s checklist for Snohomish County. 


A letter of Intent to apply for the RFP must be submitted by 4:00 p.m., Thursday, May 7, 2020. 


Organizations not submitting a letter of intent by due date and time will forfeit their right to apply for this RFP.  Letters of intent to apply for RFP are to be submitted via email to Aime Fink, aime.fink@snoco.org


Final Proposals are due Thursday, May 28, 2020 by 4:00 p.m. , local time.
